No Child Left Behind lets each state establish its own definition of "proficiency" — the target that must be reached by 2014.  Minnesota has defined proficiency as 100% of students scoring at or above Achievement Level 3 on the MCA's. The target stayed the same for the first three years (2002-2004); after that, the target is raised until 100% of students reach proficiency.
